Job Description
Tufts Medical Center seeks a Pulmonary Disease Navigator to coordinate care for patients with complex lung conditions. In this role, you will guide patients and families through diagnostics, treatment, and follow-up, ensuring seamless communication between pulmonologists, primary care, respiratory therapy, and other specialists. You will track test results, support care transitions, facilitate education on disease management and therapies, and identify barriers to adherence. The Navigator will use data and EMR tools to monitor outcomes, support quality improvement, and contribute to patient-centered, evidence-based pulmonary care initiatives.
