Provides patients and family members with the education and advice needed to cope with issues that arise from various emotional or physical problems. Coordinates a variety of services such as crisis intervention, bereavement, client advocacy, referrals, prevention, education, and psychosocial assessments. Job DutiesConducts and documents detailed psycho-social assessments/evaluations to interpret social, environmental, and medical needs of the patient and family.Manages length of stay by creating a successful discharge plan and by proactively identifying and mitigating issues and barriers to patient care.Provides supportive counseling and advocacy to assist patients and/or families with adjustment associated with illness, hospitalization, and/or alternative care placement.
