Adapt Integrated Health Care is looking for a Housing Navigator to join their team!

The Housing Navigator will be instrumental in connecting with clients within the .370 Aid and Assist population with housing options to include short term and long-term stable housing. assistance, case management, and referrals for CWP clients to remove barriers to obtaining housing. This will include helping clients identify barriers, help clients with accessing resources, and teach skills needed to navigate systems to maintain stable housing. The Housing Navigator will also provide assistance with applications/paperwork and expectations of the organizations and landlords involved in the client's case. Housing Navigators will also work on issues identified by the courts and treatment teams that relate or contribute to the client's housing needs.

Primary Responsibilities;

  • Collaborate with Community Housing Partners , property management companies, private landlords to form effective working relationships to identify, secure both short term and long term housing options for the .370 population.
  • Collaborate effectively with the court system and treatment teams to identify and address housing needs and barriers to housing.
  • Provide case management assistance in completing housing application, applying for benefits, etc to secure housing.

Qualifications


Required Qualifications:

  • Bachelor's degree and preferred two years' experience in a social service and/or mental health setting or a satisfactory equivalent combination of education, experience and training. At time of appointment, must possess valid Oregon driver's license.
  • Knowledge of basic psychological theory, principles and practices of mental health treatment including assessment, diagnosis and treatment of mentally ill persons; human behavior as it relates to crisis situations and problems of client population; substance abuse treatment as it pertains to the mentally ill, considerable knowledge of behavior intervention, management and support strategies; sound case management; knowledge of community resources including the functions and procedures of other social service agencies and service providers; education and ability to develop skills groups and workshops including planning, implementation, and evaluation of outcomes; reasonable knowledge of educational theories and techniques; reasonable knowledge of psychoactive medication and treatment applications.
  • Ability to communicate effectively in both written and oral communications that promote teamwork and collaboration with individuals served; written reports and correspondence that is both professional and informative; documentation that is clear and concise; use of motivational interviewing and other communication techniques and principles; an understanding of individual and group dynamics.
