Summary:
Provides psychosocial assessment, care planning, intervention and evaluation to individuals and families undergoing treatment for Emergency Department patients and UNC Health Nash.
Responsibilities:
- Provides care management services in the emergency department setting. Works collaboratively with other care team members to develop and support the patient's care plan.
- Identifies barriers and develops safe dispositions. Serves as a patient advocate, supporting shared decision-making and the use of decision aids.
- Educates patients and families on a variety of topics including how to access medical home services, establish self-management goals, and navigate the health care system.
- Identifies, coordinates and helps to align patient care and community resources to facilitate patient care transitions and prevent hospital readmissions and Emergency Department visits.
- Appropriately documents consults and interventions in the patient's electronic medical record.
- Maintains compliance with organizational policies and procedures and evidenced based guidelines.
Other information:
Master’s Degree in Social Work preferred.
LCSW preferred, must obtain within 3 years of hire
Minimum of two years paid experience in social work
Experience in clinical/medical setting and crisis intervention
