OBJECTIVE/PURPOSES

Under the direction of the Clinical Manager, the Therapeutic Activity Program Coordinator is responsible for programs that involve Direct Support Professionals providing community skills and respite services to clients. This includes all individual services, as well as group services, such as the Burgess Road After School Program and Camp Be a Kid in the summer. The Coordinator is responsible for supervision and assignment of clients to staff, as well as supporting staff in managing their schedules and required administrative paperwork.

MAJOR RESPONSIBILITIES:

  1. DIRECT SERVICE:

The Coordinator will assist with groups and fill in for Direct Support Professionals as needed. The Coordinator will hold a small caseload providing community skills and/or respite services to clients.

  1. RECORD KEEPING:

Completes agency paperwork including, but not limited to, submitting accurate daily and weekly time sheets, expense sheets, and monthly documentation in a timely manner. The Coordinator will complete the quarterly respite report for the Department of Mental Health.

  1. NETWORKING:

Provides pertinent information to other staff, professionals, and families in a professional and confidential manner. Serves as an advocate and/or liaison on behalf of individuals served, and UCS. Participates as a problem-solver in team and other meetings. Holds regular supervision with all supervisees.

  1. CONSULTATION

Provides consultation/updates to case managers, as needed about child's needs/behavior. May attend treatment team meetings, as needed. Reviews the CANS assessment for all referrals to determine if the Therapeutic Activities Program is appropriate.

* In addition to the above, performs other duties as assigned.



Qualifications:

EDUCATIONAL/EXPERIENCE/CREDENTIAL REQUIREMENTS:

A Bachelors Degree in a relevant field or at least four years of relevant experience in the human services field is required.

LICENSE REQUIRED:

Valid Driver's License

ACCESS TO PROTECTED HEALTH INFORMATION (PHI)

In order to perform the functions of my position, I understand that I have access to the clinical records of:

D The persons I serve directly, for creation and review of notes that support service delivery,

D The persons that I may serve while I back up a member of my team, for creation and review of notes that support service delivery,

D The persons that I may serve in a crisis or emergency situation, for creation and review of notes that support service delivery,

D The persons specifically authorized by my supervisor.

I understand that I have access to UCS' client billing software according to the profile on file with the Information Technology department.

I understand that I do not have the right to remove or destroy any information contained in clinical records at any time and that protected information created by me will be included in the clinical record in a timely manner according to UCS' documentation standards and policies. I understand the components of the designated record set as explained.

I understand that I do not have the right to remove clinical records from the office, unless I have specific and time-limited authorization to do so, for treatment meetings, for the purpose of obtaining signatures of persons served, or for other work related purposes that have been authorized by my division director.

I understand that whenever I have protected health information in my possession, electronically or otherwise, it is my responsibility to safeguard that information. This means that I will password protect or lock the information in my car or keep it in my possession at all times, rather than leaving it unattended or in plain view and destroy any PHI contained in any personal electronic communication device. See Compliance policies in the Section 10 of the UCS Policy and Procedure manual.

I understand that if I access consumer information unnecessary for the requirements of my job, without authorization, I will be subject to the procedures outlined in employee policies and as it applies, the Corrective Action policy.
