Short Description

Navigator Specialist Program Coordinator, Perinatal will use professional experience and expertise in both patient engagement and administrative coordination to support Cooper's perinatal substance use and recovery program.

Patient engagement will account for approximately 70% of the time, with Administrative work accounting for 30% of time, although this may change depending on program needs.

Provide ongoing care-coordination and case management services to pregnant and parenting patients with SUD.

Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services.

Provide education, coordination, navigation, linkages, and support to pregnant and parenting individuals with Substance Use Disorder

Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome

Provide continuity of care by connecting patients to various clinical care settings including the following:

emergency department visits, addiction treatment appointments, OB/GYN appointments, social service appointments, and therapeutic appointments.

Assist with acquisition, distribution and documentation of resources through the state EMPOWR grant, liaise with grant administrative staff.

Assist in developing patient level documentations (contracts etc) for engagement in EMPOWR grant (i.e. daycare, emergency housing, intake/discharge etc)

Assist in development of onboarding training materials, patient education materials, and continuing education

Engagement of grant eligible patients in both inpatient and outpatient services. Linkage to outpatient care and grant resources during inpatient hospitalization.

Maintain positive relationships with external resources

Attend Addiction Medicine clinical and programmatic conferences and meetings as required.

Participate in regular program/grant meetings.

Engage in ongoing professional development, training and receipt of clinical supervision.


Education Requirements

High School Diploma required

Bachelors' degree in a related field preferred, plus 1 year of relevant experience working with patients with Substance Use Disorder

OR Associates degree with 2 years of relevant experience

OR Certified Peer Recovery Specialist designation plus 2 years relevant experience.

OR Certified Medical Assistant with 4 years of relevant experience.


License/Certification Requirements

CMA Preferred

CPRS Preferred
