A Brief Overview

The Children's Hospital of Philadelphia, Division of Human Genetics, Section of Metabolism, Lysosomal Storage Disorders Program is seeking a Nurse Practitioner or a Physician Assistant. The ideal candidate would adeptly blend his/her experience in clinical care, research, and teaching in the diagnosis and management of patients with lysosomal storage disorders (LSD) and inborn errors of metabolism.

In collaboration with an attending physician, the Advanced Practice Provider is responsible for the direct care and follow-up of patients with metabolic disorders that matches his/her scope of practice and skill set.

The successful candidate should have an interest in metabolic disease and be willing to acquire additional knowledge by personal study, interaction with colleagues, and didactic teaching courses offered within the hospital and national meetings. In addition, having a basic understanding of the principles of inheritance is desirable. Candidates with demonstrated experience in managing complex patient cases within an academic healthcare setting will be preferred.

Responsibilities

  • Preparation for clinic visits - abstract recent information for new and follow-up patients from CHOP electronic medical records by reviewing consultations and evaluations by each specialist since the patient's last visit to the LSD or Metabolic clinic & interim history supplied by patient or non-CHOP medical staff.

  • During clinic, obtain updated history from patient, order/coordinate appropriate laboratory and imaging studies for disease monitoring. Discuss updated plan of care with patient/families, in collaboration with attending physician and other multi-disciplinary providers involved in clinic visit.

  • Communicate results of biochemical testing and imaging studies, as well as any changes to the medical plan with patient/family.

  • Order and manage initiation of, and changes to, enzyme replacement therapies for patients in CHOP Day Medicine, external infusion center, and home infusion settings. This may include communication with authorization teams, pharmaceutical case managers, patients' insurance, and participation in peer-to-peer discussions and appeals process as necessary.

  • Remain available to communicate with Day Medicine team, families, and patients regarding concerns. See patients in Day Medicine as needed and maintain documentation of patient encounters.

  • Collaborate with Genetic Counselor to coordinate genetic and biochemical testing, as well as extended familial testing as appropriate.

  • Oversee management of LSD patients lists in Excel: demographic data, genotype, treatment (include date initiated), date of last visit, dates for future tests (esp. radiologic) and name of LSD physician

  • Organize local meetings with patient support groups regarding LSDs; including logistics, publicizing/inviting attendees, presentation preparation and obtaining grant funding.

  • Participation in various clinical trials, research studies, and patient registries alongside research coordinators and other clinical staff.

  • Manage a cohort of patients, perform and document daily assessments, collect data, and analyze findings to inform family-centered plans of care.

  • Perform advanced procedures listed in the Scope of Practice and approved as a part of the credentialing process.

  • Serve as a consultant and educational resource to patients, families, students, and multidisciplinary healthcare team members.

  • Participate in scholarly activities and maintain required competencies, identifying learning needs and seeking appropriate educational offerings.

  • Address safety issues and demonstrate shared accountability for safe practice.

Education

  • Required: Master's degree- Nursing

  • Preferred: Doctorate- Nursing

Education

  • Required: Bachelor's Degree - Physician Assistant

  • Preferred: Master's Degree - Physician Assistant

Skills and Abilities

  • Ability to work in a team setting with multiple responsibilities

  • Ability to use computer and electronic medical records effectively

Licenses and Certifications

  • Required (upon hire in Pennsylvania): Registered Nurse and Certified Registered Nurse Practitioner (CRNP) (Pennsylvania) - Pennsylvania State Licensing Board

  • Required (upon hire in New Jersey): Registered Nurse and Advanced Practice Nurse (APN) (New Jersey) - New Jersey State Licensing Board

  • Required (upon hire): Specialty Certification as a Nurse Practitioner in area of education/subspecialty

  • Required (within 3 months): Prescriptive Authority - Pennsylvania State Licensing Board or New Jersey State Licensing Board (depending on location)
