Short Description
Provides clinically competent case management services to clients as needed |
Includes phone calls, communicating with patients and families, and completing forms or letters for the physicians. |
Provide referrals and linkage to community-based services, and case management for patients and their families. |
Contribute to the development of individualized plans of care, and encourage the compliance with treatment plan goals with respect to case management objectives. |
Participate in daily clinical team meetings and document updates in client records. |
Documentation of all encounters is completed in an accurate and timely procedure within program policies and procedures. |
May coordinate care with outside treatment providers and care management organizations. |
Extract information from our EMR and utilize it to identify referral sources, time from referral to intake, and various other benchmarks. |
Complete reports at least monthly, working with clinical supervisor. |
Other duties and/or projects as assigned. |
Experience Required
0-2 years
Education Requirements
Social Services/Health Field, such as, but not limited to: Social Work, Psychology, Nursing |
BA/MA may be in another closely related field if 2+ years in related job. |
