Position: LPN or RN, Behavioral Health

Primary Care is a Federally Qualified Health Center with 11 clinics in 5 parishes
Job Overview: Our sites qualify for the National Health Service Corps loan repayment
Direct Patient Care:
- Verify patient information, obtain vitals & document in EMR
- Interact with patient to obtain medical history & document in EMR
- Ensure all reports/labs are available
- Conduct motivational interviews
- Internal and external referrals
- Follow-up on cancelled/no show appointments
- Coordinate prescription refill and medication administration
- Perform medical assessment and interventions
Qualifications/Skills:
- Current LPN/RN license in good standing with the State of Louisiana (Required)
- Current Basic Life Support (BLS) (Required)
- Prefer experience in behavioral health care
Employment Type: Full Time, Monday through Friday

Starting Salary Range: LPN - $18.66 - $24.35 per hour; up to 3% annual increase
RN - $50,230 - $71,240 per year; up to 3% annual increase
