Engages with participants in a collaborative process to assess health status and develop personal goals and action plans that promote wellness and help individuals realize their personal best.Job DutiesProvides telephonic, in-person, and/or virtual health coaching by assessing individual needs and readiness to change using coaching models and theories, such as motivational interviewing, to assist individuals in creating personalized health and wellness goals.Documents wellness coaching encounters including appointments, outreach, and referrals in accordance with program, NCQA, and department guidelines to coordinate care across the continuum and encourage direct communication between the individual and their primary care provider or other appropriate network or community resources.Develops, promotes, and provides evidence-based health education and wellness offerings such as presentations, webinars, programs, biometric screenings, and results consultation to internal and external clients and partners.Assists in the monitoring, evaluation, and improvement of the BeneFIT health coaching and wellness programs to meet program goals.Acts as a wellness and health education resource to colleagues, clients, brokers, payers, and community partners; develops and maintains relationships with stakeholders internally and externally.Attends appropriate trainings and seminars to maintain coaching and other professional standards/certifications; expands knowledgebase and experiences.Minimum QualificationsBachelor's Degree in nutrition, nursing, kinesiology, health education, community health education, public health, or psychology. 2 years of experience working as a health and wellness coach and2 years of experience using readiness to change, motivational interviewing, goal setting, and resolution techniques to assist participants in developing individualized plans for successful behavior change. Ability to manage multiple calendars and consistently meet deadlines. Ability to creatively problem solve and work within a self-directed work team environment. Demonstrated ability to locate and link individuals to network and community healthcare resources. Excellent computer skills including MS Office, Word, Excel, Outlook, and PowerPoint. Ability to interact with a wide variety of individuals and corporate environments including business and industry, internal staff, hospital departments, community organizations, and health insurance companies. DL - Driver's License_PA - State of Pennsylvania Upon Hire Preferred QualificationsMaster's Degree in nutrition, nursing, kinesiology, health education, community health education, public health, or psychology and training in motivational interviewing, active listening skills, and application of the transtheoretical model. Bilingual (Spanish and English).
