SUMMARY

This position is for a clinical pharmacist embedded within the Cardiology 2 days per week (in Needham) and Pulmonary 1 day per week (in Boston at Kenmore) clinics, with remote work from home 2 days per week. Under general supervision provides clinical pharmacy support to clinicians and staff at these medical practices. Is responsible for optimizing patients’ health outcomes and use of guideline directed drug therapy through Collaborative Drug Therapy Management (CDTM), direct patient care, drug information consultation, academic detailing, cost-savings/quality prescribing initiative rollout, and clinician/staff education at practice(s) of responsibility. Is responsible for central clinical pharmacy support through participation in committees and workgroups, participation in and/or management of standard department programs and services, and participation in the development of organization-wide drug therapy management materials and tools.

EDUCATION/LICENSES/CERTIFICATIONS

Graduation from an accredited school of pharmacy. Must possess an advanced degree (i.e., PharmD), and have completed a post-graduate year one (PGY-1) residency (preferably in ambulatory care clinical pharmacy) or have equivalent ambulatory care clinical pharmacy experience. Must possess advanced training in drug therapy management related to Cardiology and Pulmonology, such as a post-graduate year two (PGY-2) specialty residency or work experience or training. Licensed as a Pharmacist to practice in Massachusetts. Must meet or be eligible to meet all criteria for CDTM per Massachusetts Board of Pharmacy regulations. American Heart Association Basic Life Support (BLS) required. Advanced Cardiac Life Support (ACLS) may be required based on specialty.

EXPERIENCE

At least 5 years of pharmacist licensure and at least two years of clinical pharmacy experience, or 1-year clinical pharmacy residency required. Additional experience required in Cardiology and/or Pulmonology.

SKILLS

Possesses excellent communication skills, both written and oral. Displays well-developed interpersonal skills to work successfully with all levels of the organization and patients.
