Qualifications:
The Clinical Care Coordinator implements, supports and helps coordinate the services for chronically mentally ill adults living in the community in conjunction with their individual treatment plans.
MAJOR RESPONSIBILITIES:
CONSUMER SUPPORT: The Clinical Care Coordinator provides Clinical Care Coordination, supportive counseling and skill building for assigned clients. This is a community-based position, which means that you will be meeting the client in the community rather than having the client meet with you in an office. Supports and coordinates services for clients. This position may have clients in other areas of Bennington county and will need to travel to the client. Provides assistance in getting income support and entitlement. Helps to foster peer supports. Facilitates growth of community and family support. Facilitates assistance to meet necessary mental and physical health needs. Participates in all aspects of the Outpatient Division.
May be assigned to provide Substance Use Disorder (SUD) services, or within related SUD grants. The incumbent will show competency in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA and 42 CFR Part 2, and other evidence-based counseling and professional ethics theories, in order to provide effective care.
CONSUMER IDENTIFICATION AND OUTREACH: The Clinical Care Coordinator provides outreach services to all in the community who are in need of our services. Develops systems to provide services to area clients. Ensures protection and advocacy of client's rights whenever possible. Provides clients with referral information and assists with transportation as needed. The Clinical Care Coordinator would facilitate a non-client's ability to access our intake process.
INFORMATION MANAGEMENT AND RECORD KEEPING: The Clinical Care Coordinator completes various clinical and administrative forms for billing, information and clinical records. Completes all case management documentation within 24 hours and finalized within 3 business days and in a professional manner. Attends all staff meetings, agency meetings, in-services, and clinical trainings as instructed by supervisor. Maintains confidentiality on all cases at all times. Follows goals of the program and policies of United Counseling Service.
COMMUNICATION: The Clinical Care Coordinator ensures that all persons involved with provision of services and support to the client receive appropriate communication about the care and planning related to the client.
REQUIRED QUALIFICATIONS:
Minimum qualifications of a high school diploma and training in Motional Interviewing and Collaborative Network Approach modalities. Preferred Bachelor's degree in Social Work, Psychology or related Human Services field or equivalent or experience of 4 plus years
