Job Summary:
We are seeking aClinical Services Nurse IIinEndocrine Triage at the Duke Health Engagement Centerto be aprofessional telehealth registered nurse caring for adult patients referred to the DHIP Diabetes Center of Excellence transitional care program via phone or other technological means. The mission of the program is to improve patient health outcomes, patient experience, and reducetotal cost of care related to high-risk patients with diabetes and Diabetes related complications.The nurse assesses patient needs, develops, and implements a plan of care, provides appropriate clinical dispositions to providers, facilitates specialty referrals, schedules patients for various types of clinic appointments, and offers appropriate internal and external resources. The nurse also triages urgent/emergent patient phone calls from a nurse phone que, advises and educates patients on appropriate health care setting.
Job Duties and Responsibilities:
- Care coordination and follow up after hospital admission for diabetes related concerns
- Provide education to patient related to their diagnosis and treatment including insulin administration
- Identify care gaps, patient trends, readmission prevention interventions and barriers to compliance with program.
- Support ongoing expansion of services and continuous improvement
- Coordinate with providers both inpatient and outpatient across the care continuum.
- Perform other related duties incidental to the work described herein.
Job Eligibility Requirements:
- Work requires graduation from an accredited BSNprogram.
- 3-5 yearsexperience in acute care nursing
- 3 years experience in ambulatory care. Endocrine Specialty preferred
- Must have current or compact RN licensure in the state of North Carolina
- Diabetes Educator Certification preferred
- Ambulatory Certification preferred
Job Hours:
The schedule for this position is day shifts, Monday-Friday, with no weekends or holidays.
